NO ONE WALKS ALONE, WE WALK WITH YOU

SUPPORTING CAREGIVERS PLANNING FOR WHEN THEY ARE GONE

The last year or so we have been exploring ways we can support caregivers planning for when they are gone or are no longer able to provide for the required supports. We listened to caregivers – many aging – about their fears and plans for their loved one’s future.

WHAT WE HEARD

Caregivers (and the person they care for) are ill-prepared for the future – they want support:

  • To put plans in place for the future

  • For a range of planning needs

  • To implement future care plans

  • That fosters trusting long-term relationships

THE PATH FORWARD

Based on what we heard, we are now urging the Ontario government and others to support a Caregiver Future Planning Pilot Project which will inform our efforts around the development of a service delivery model that is all about community and caregiver engagement in support of those we (caregivers) care for living with mental illnesses and/or substance use disorder – to learn more go to: The Path Forward
